(The Community Falls Service is expected to achieve a 10% reduction in the rate of emergency admissions related to falls in each year of the contract (baseline 2014/15 outrun)). The annual contract value will be capped at a maximum of no more than £142,000. The maximum total contract value would be a maximum of £710,000 (including the possible extensions.)

   Description of the circumstances which rendered the modification necessary and explanation of the unforeseen nature of these circumstances:
    A procurement for the provision of a Postural Stability Instructor community-based provision was unsuccessful in 2014-15. This was unforeseen and prompted the Council Public Health team to run this service in the first instance. The PSI market in Southend remains the same; to avoid a further unsuccessful procurement and capitalise on any overlap of service this community-based PSI provision can be provided alongside the acute community falls provision under the one contract.
